Please use the 2011-2012 AYP/SI Appeal form to complete an appeal for AYP/SI Status.
Submitting this form completely and accurately is mandatory for an appeal to be considered. Fill in all required information (Sections A – E) and mail the completed form, along with all necessary supporting documentation, to:

The AYP/SI Appeal Form and all supporting documentation for your appeal must be received by ODE via mail by Wednesday, Aug. 1, 2012. Please note that appeals submitted via fax and/or e-mail will not be considered by ODE.
All information pertaining to your appeal must be received in one document; under no circumstances will incomplete appeals be entertained. If you have multiple appeals to submit, you must fill out a separate form and appropriate documentation for each one. Districts will be notified of all appeal decisions via e-mail by Wednesday, August 8, 2012.
