FY13 Graduate Reporting Period Re-opened
The FY13 Graduate (G) reporting period has been re-opened today.  The new closure date is Friday, June 13, 2014.  Districts only need to resubmit if they have a student who is an error from the district’s last submission.
Submit 2013-2014 Graduation Rate Adjustment Form by May 30, 2014
The 2013-2014 Ohio School Report Cards will include both the Four-Year Adjusted Cohort Graduation Rate for the graduating class of 2013 and the Five-Year Adjusted Cohort Graduation Rate for the graduating class of 2012. Both rates will be displayed on the report card and used in accountability calculations.

There are cases in which some schools and districts may still be able to make adjustments to their cohort data. Those needing to request an allowable graduation cohort adjustment should submit a 2013-2014 School Year Graduation Rate Supplementary EMIS Data Submission Form. Allowable adjustments generally are limited to situations in which the student (or parents) reported doing one thing and then actually ended up doing something else. Adjustments for coding errors and other record keeping mistakes cannot be accepted.

Click here to download the form, instructions and clarification on acceptable adjustment requests. Submit the fully completed form to the department of education via e-mail at Only one student and one adjustment request may be included on each form.
