Guidance on Documentation of Transition Practices

With support from the federal Preschool Development Grant (PDG), a cross-agency team led by Ohio Department of Job and Family Services (ODJFS) has been working to identify, examine, and leverage best practices for children’s transitions. Based on information many of you provided, the following guidance has been created to reduce administrative burden and provide clarification regarding the use of transition plan documents.  
 
The JFS 01373 “Transition Plan for Three- to Five-Star Ratings for Step Up To Quality (SUTQ)” meets the minimum requirements for SUTQ and Head Start. The form can be found at: http://www.odjfs.state.oh.us/forms/.
This is a sample form and all elements on the form are required. However, use of this form is not required for Head Start. Programs can meet the transition plan requirements for SUTQ and Head Start compliance in one of the following ways:
 
  1. Use the ODJFS sample form in your program, as it is. 
  2. Use the ODJFS sample form and attach a separate document for any additional Head Start or individual program requirements.
  3. Recreate the ODJFS sample form and customize it with your own branding and any additional elements needed to meet local Head Start or individual program requirements. 
  4. Continue using your own forms, making sure that information on the ODJFS sample form is included.
 Licensing Specialists from Ohio Department of Education and ODJFS are informed of this guidance and are available to provide technical assistance on licensing and SUTQ transition requirements.  If you have questions specific to your program, please contact your Specialist.
