Striving Readers Impact Stories

The Ohio Department of Education is collecting information and success stories about the Striving Readers Comprehensive Literacy grant and its impact on students, teachers and districts. The lead district/consortium should complete the survey on this webpage. 

This survey will not save until submitted. It is recommended that you compile your data and responses in a separate document and copy and paste your information into the fields or set aside enough time to complete the survey in one sitting. This survey may take 45-60 minutes to complete.

To prepare for the survey, you will want to access your most recently submitted compliance survey. This information will be helpful in answering many of the questions.

If you need assistance or have any questions as you complete the survey, please refer to the guidelines below, review the Impact Story Examples shared or contact your individual technical assistance provider.


Please type the full name of the LEA that serves as the consortium lead or individual subgrantee as you would like it to appear, for example, please write Western Local School District, instead of just Western.

Please list the full names of all LEAs that are members of your consortium. If you are a single district, leave this field blank.


For the next five responses below, please provide the most accurate numbers you can for each of the prompts for the 2018-2019 school year. You may use your most recently submitted Appendix 3 as a starting point and make any necessary adjustments.

Select four key grant activities to highlight here. Be sure activities are in chronological order, as this is the order they will appear on the timeline on the template. For example, Activity 1 would begin before Activity 2. Some examples of possible activities can include professional development offerings, implementation of evidence-based practices, family engagement activities, etc. Ideally, these would be activities that align with each of your grant goals. These do not need to be all-encompassing – not all activities need to be represented by the four activities highlighted here. Fill in a brief, one-sentence description of the activity. In general, it is too early to say with certainty whether Striving Readers funding is leading to improved student assessment outcomes. Your statements of impact should not focus on assessment outcomes - but there are many ways you can consider impact. For example, use this as an opportunity to share funding highlights. Tell us about changes in attitudes, knowledge or behavior you have observed in your teachers and students as a result of the funds and activities implemented through this grant. Are there opportunities for training or support that now exist as a result of the Striving Readers funds? For this question, focus on changes you can measure and connect directly to the Striving Readers funds.

Describe the potential impact of continuing the work, even after funding ceases at the end of Year 3 of the grant. What are the long-term impacts and how is this opportunity building capacity? For example, continued development of literacy coaches in elementary buildings, increased capacity to implement evidence-based practices, etc.

Gather and list one or two brief quotes from individuals impacted by Striving Readers funding. These quotes can be from building administrators, teachers or students. These quotes should not be from regional or central office staff. Consider asking individuals how Striving Readers has supported ongoing work, how Striving Readers has affected their instruction or how Striving Readers has enhanced their own personal knowledge.
