K-12 Notice of Intent

Commitment to return to in-person or hybrid education delivery model by March 1, 2021; forms must be submitted by Jan. 18, 2021, to schools@odh.ohio.gov

Students and educators across the state have done a great job of complying with recommended health practices since the COVID-19 pandemic began in an effort to keep one another safe. Now, as safe, effective COVID-19 vaccines have become available, Ohio has embarked on a plan to distribute them statewide in a phased approach. During Phase 1B, vaccine supply is limited, and vaccine administration will be focused on reaching critical groups, which include adults who work in schools. Ohioans should be aware that, due to vaccine shortages, anyone who declines the vaccine at their first opportunity may not have another chance to receive it for some time. Governor DeWine announced that vaccinations will be available for personnel in Ohio schools starting the week of Feb. 1. For school staff members to be prioritized to receive the COVID-19 vaccine at that time, their districts or schools must have alerted the Ohio Department of Health of their commitment to return to in-person or hybrid education delivery models by March 1, 2021.

As more details emerge about the K-12 School Staff Vaccination Program, this webpage will be updated with important resources and information. In the meantime, it is critical to continue following guidelines to wear masks, social distance, wash hands and limit travel and large gatherings.
