Survey Tool to Replace Child Count for County Board of Developmental Disabilities
In prior years, the County Boards of Developmental Disabilities (DD) submitted child count through an Excel spreadsheet during the first full week of October to the department. This was utilized to validate the correct count and calculate the state funding amount for each DD.
The Department has developed a new survey tool which was sent to DD’s to collect this information. This new web-based survey method should prove to be easier to complete and submit to the department.
The information reported reflects the number of students with disabilities who are served by your CBDD during the first full week of October 2013, in accordance with an Individualized Education Program (IEP) currently on file. By submitting this data you certify that the information provided on this form is, to the best of your knowledge, complete and accurate. A knowingly false claim in this report is a criminal offense under U.S. Code, Title 18, Section 101 or Section 287.
