OGT Breach Forms

The use of a breach form may be approved by ODE if a student's test has been compromised by a test irregularity or a test security violation. DTCs should call the Office of Curriculum and Assessment to discuss the situation and obtain approval for use of the breach form. Districts may not request breach forms directly from AIR, DRC or the OGT Help Desk. Note that costs associated with breach incidents resulting from district error may be charged to the district.
