School food authorities interested in participating in the Community Eligibility Option (CEO) for one or more schools must complete the following two documents and submit them to Diane Schneider by June 30, 2013. Forms may be faxed to 614-752-7613 or emailed to diane.schneider@education.ohio.gov.

LEAS that have elected to participate in CEO for at least one site must notify households of children attending the CEO school(s) of CEO participation in the 2013-2014 school year. LEAs may use this sample household letter to serve as the required notification.
