Annual required immunization reporting due Oct. 15

Ohio law requires all schools to report an annual summary of the immunization status of pupils by Oct. 15 to the Ohio Department of Health. Each school with an Information Retrieval Number (IRN) must submit a report.

The Ohio Department of Health’s Immunization reporting webpage provides online instructions, worksheets, and a reporting tool.
