Consolidated School Report
9/24/2024
Consolidated School Report opens Nov. 1
Ohio law requires traditional public school districts, including joint vocational school districts, to annually report on a specified set of data to the Ohio Department of Education and Workforce through the Consolidated School Report. Districts are required to report data from the 2023-2024 school year through a report survey in the Data Collection application within the OHID portal. The reporting window is open Nov. 1-30.
This consolidated reporting, which collects the required information at one time and in one tool, includes the following:
- Training on the use of physical restraint and seclusion
- Training on harassment, intimidation, and bullying
- Training on cardiopulmonary resuscitation (CPR) and automated external defibrillators (AEDs)
- Compliance with nutritional standards
- Screening for hearing, vision, speech, and communications
- Health or medical problems and developmental disorders
- Intradistrict and interdistrict open enrollment policies
If the school district fails to complete the report, reports noncompliance by answering “no” to any question, or fails to upload the nutrition report, it must provide a written explanation to its board of education within 30 days. The written explanation should detail why each item was not compliant and a written plan of action for accurately and efficiently addressing the problem.
Those identified in the Ohio Educational Directory System (OEDS) as superintendent or superintendent designee for the district have access to complete the report. Using the Chrome internet browser is recommended to complete the report.
Please note, community schools and nonpublic schools do not report this data.
