Community School Sponsor Application

Following is the sponsor application available from the Ohio Department of Education and Workforce for entities and organizations interested in becoming community school sponsors in Ohio. This application and review process will be used for the 2024-25 sponsor application cycle. Eligible entities and organizations should then complete the application below and submit it, along with all required documentation, to the Department through Epicenter, a data collection system, by December 31, 2024.

Note:  Only those entities and organizations that meet the eligibility requirements listed on the sponsor application should apply to become a sponsor.
