Medical Waiver Reminder
Ohio law requires students to participate in state assessments for grades 3–8 and end-of-course exams. In rare cases, a significant and unexpected medical emergency may prevent a student from testing. When this happens, districts and community schools may apply for an Emergency Medical Waiver

The application to apply for a medical waiver will be available in the Data Collection tile in OH|ID from May 11–29, 2026. Superintendents or their designees must submit the completed application by the end of business on May 29, 2026. Incomplete applications will not be reviewed.

For students for whom a medical waiver will be requested, a Student Assessment (FA) Record should still be reported for the relevant assessment(s). Report the most appropriate Score Not Reported Option (see EMIS Manual Section 2.8: Student Assessments). If the waiver is approved by the Department, the student will be removed from the local report card participation rate and other calculations based on the assessment(s). If the waiver is not approved by the Department, the student will not be removed from these calculations and will be included as “untested”.
