This form is intended exclusively for submitting requests to generate new reports compiled from data maintained in the Department’s databases.

Please note that under Ohio’s Public Records Act, public offices are not required to create new records or compile information from existing records unless the requested output already exists through standard reporting functions. If you are seeking copies of existing records or documents, please direct your request dewrecordsrequest@education.ohio.gov.
