Driver Insulin Waiver
At times drivers that are seeking to transport students in Ohio might be required to obtain an insulin waiver. In order to transport students, a T-8 physical is required. The driver will inform the physician of any medical condition(s) they may have. The treating T-8 physician will then complete the T-8 form based on their findings. If the T-8 is marked as a driver needing to obtain an insulin waiver, they can download the insulin waiver packet and have it completed by a licensed endocrinologist and a licensed ophthalmologist.
Pursuant to
OAC 3301-83-07 a driver will need to intially submit the insulin waiver packet to the department that includes the following:
- Endocrinologist approval form
- completed and signed by the licensed physician with license number and date
- only physicians are licensed as endocrinologists (not APRNs or Physician’s Assistants)
- Ophthalmologist approval form
- completed and signed by the licensed physician with license number and date
- Application
- With the drivers’ signature and date
To maintain a valid waiver, a driver is to submit an endocrinologist approval every six months and an ophthalmologist approval annually to their employer.
