Driver Missing Limb Waiver

At times drivers that are seeking to transport students in Ohio might be required to obtain a missing limb waiver.  When a driver attends their T-8 physical to transport students they are required to inform the physician of any medical condition(s) they may have. The treating T-8 physician will then complete the T-8 form based on their findings. If the T-8 is marked as a driver needing to obtain a missing limb waiver, they will need to obtain the waiver packet and request a performance evaluation to be conducted by Ohio pre-service program instructor(s) to determine the person's ability to control and safely operate a school bus or school motor van.
