The Ohio Department of Education & Workforce has proposed revisions to the state required and supplemental (formerly optional) forms for special education to streamline and clarify expectations of state and federal requirements. As part of the process, the Department is seeking input from stakeholders across the state on the proposed revisions.  Please click here to see proposed changes and links to provide feedback.

The Department of Education and Workforce invites you to participate in a series of feedback sessions on the revised special education forms. Your insights and feedback are invaluable to us as we strive to enhance the clarity and effectiveness of these forms. All sessions will be recorded and posted on this webpage, please only attend if you can be present and participate in the meeting, registration is limited. Proposed Form Revisions

The Ohio Department of Education & Workforce is in the process of updating Ohio’s special education required and supplemental (formerly optional) forms.  As part of the form revision process, the Department is seeking input from stakeholders across the state on the proposed revisions using Microsoft Forms surveys. Please see below for additional information.

Please compare the current forms and the proposed revised forms, either individually or using the side-by-side comparison documents below, prior to survey completion. You may optionally identify yourself by email within the survey. Below each section (required and supplemental forms) you will find a Microsoft Form survey link. External Stakeholder feedback may be given at any time using these survey links, until May 23, 2025.

Required Forms

Here is a side-by side comparison document of the required forms.  This document provides a side-by-side comparison of all changes made to the current required forms. This comparison is meant to provide a broad visual overview of the changes; to view a full-size copy of the current form, click on “Current Form” or to view the full-size copy of the proposed revised form, click on “Revised Form.”

OP-3: Manifestation Determination
PR-03 Manifestation Determination Review
The updated PR-03 form combines elements of the old PR-03 with the optional OP-03 form.  These changes can be seen in the differences between existing sections of the PR-03 as well as the inclusion of new sections from the OP-03 form. The OP-03 Form will no longer exist.
PR-04 Referral for Evaluation
The PR-04, PR-05 and PR-06 were combined into a single ETR package containing Referral Information and Parent Consent for Evaluation.

Supplemental Forms (Formerly Optional Forms)

The optional forms have been renamed to supplemental forms.  This was changed to provide more clarity on the use of these forms. 
Here is a side-by side comparison document of the supplemental forms.  Below is a table for a quick reference with links to all current required forms along with the proposed revised forms.

OP-3: Manifestation Determination
The updated PR-03 form combines elements of the old PR-03 with the optional OP-03 form.  These changes can be seen in the differences between existing sections of the PR-03 as well as the inclusion of new sections from the OP-03 form. The OP-03 Form will no longer exist.
SF-3 Future Planning (NEW)
The SF-3 Form is a brand-new form to assist teams by providing guiding questions and considerations for developing the future planning statement for each student within the Individualized Education Program (PR-05) Section 1: Future Planning.
OP-4: Agreement to Waive Reevaluation
OP-04 has been removed as a form is not necessary for this instance.
SF-4 Parent Consent for Additional Assessment (NEW)
The SF-4 Form is a brand-new form to assist teams in obtaining parental consent for an additional assessment outside of the evaluation process.

SF-8 has been expanded into two separate options, SF-8A for general Summary of Performance and SF-8B with expanded guidance on Summary of Performance.
Newly created SF-8B Summary of Performance Packet.  Educational agencies will have a choice of using the one-page SF-8A Summary of Performance form or using the SF-8B Summary of Performance Packet. This packet provides a way to organize all information about the student and what the student will continue to need to achieve post-secondary goals after graduation.

Virtual Stakeholder Feedback Event Details

The Department will be hosting four virtual stakeholder meetings to review proposed special education form revisions and provide an opportunity for feedback on the following dates: 
  • Virtual Meeting 1: April 24, 2025, 12:00 PM to 4:00 PM - Review proposed Individualized Education Program (IEP) forms and provide feedback in a small group breakout room or via the Microsoft Form survey. Immediately following is a review of the proposed Evaluation Team Report (ETR) forms and small group feedback session.  To register click here.  
  • Virtual Meeting 2: May 6, 2025, 10:00 AM to 12:00 PM – Review and provide feedback on proposed forms relating to parents including parent/student invitation, parent consent for additional assessment, IEP team member excusal, prior written notice, assignment of surrogate parent, and attempts to obtain parent permission.   • Virtual Meeting 3: May 8, 2025, 10:00 AM to 12:00 PM – Review and provide feedback on proposed forms relating to behavior including manifestation determination, functional behavior assessment, and behavior intervention plan. To register click here
  • Virtual Meeting 4: May 13, 2025, 10:00 AM to 12:00 PM – Review and provide feedback on proposed forms relating to student progress and growth including future planning, transition progress, summary of performance, and progress reporting.  To register click here.
