Special Education Forms Proposed Revisions

Thank you for providing valuable input and feedback to the draft proposed form revisions during the comment period. Feedback surveys are now closed, and the Department is in the process of reviewing these considerations and making revisions to the proposed forms. After those revisions are made, the forms will go through an approval process at the Department before being released to the public. There will be an implementation calendar once we have finalized forms. Please check back here for future updates.

July 2026 Update on Forms

The special education forms are currently going through the final formatting process and should be out soon. Please note that these forms are not required to be used for the 2026-2027 school year. The Department will be providing training to the field during this time on the forms. Educational Agencies will be required to use the forms during the 2027-2028 school year.

Roll-out and Training Plan

During the 2026-2027 school year, the Department will offer a variety of training opportunities for educational agencies to become familiar with the updated required and optional forms.
