Complete the 2021-2022 diabetes survey by Feb. 17

The Department reminds districts to complete the 2021-2022 diabetes survey, located on the Department’s website, by Feb. 17, 2023. Ohio law requires each school district to report annually to the Department its school nurse data on the number of students with diabetes enrolled in the district during the prior school year. Each district also must report the number of errors that occurred in administering diabetes medications to students during the same period.

All public districts and chartered nonpublic schools must complete the survey. Non-chartered, nonpublic schools do not need to complete the survey.
