2021-2022 Prevention Services Data Reporting due March 31

3/20/2023

Each year, Ohio law requires schools to report the types of prevention-focused programs, services and supports provided to staff, students and families. Traditional public schools, community schools, state supported organizations, STEM schools and career-technical education schools must complete a Prevention Services Data Report to indicate which types of prevention-focused programs, services and supports were provided during the 2021-2022 school year.

District superintendents and community school leaders in schools required to report on prevention-focused services should receive an email from Ohio K12 Help with a link to create and submit each schools’ Prevention Services Data Report. District or school designees must submit the report by Friday, March 31.
