Roadmap supports families of students with disabilities through transitions along educational journey

The Secondary Transition Roadmap is available on the Families of Students with Disabilities webpage. The Department developed the Secondary Transition Roadmap to help families of children with disabilities ages 3-21 better understand points of transition throughout a student’s education journey. The roadmap places special emphasis on transitions for children ages 14-21. The interactive map provides age-related information, resources, service agencies and planning tools for each point of transition. It also aligns with Ohio’s plan to improve outcomes and learning experiences for students with disabilities recommendation three of Each Child Means Each Child.

The transition roadmap was designed and developed with families at the center. Input and design came from partner-based teams across the state, families and other state agencies, all with a strong focus on family, caregiver and students support. The interactive map will address each stage of the postsecondary transition planning process.
